What you're looking at is an usual set of symptoms for GPU detachment which is 
an extremely common problem on T4x units.

Three fixes:

a) Replace the system board

b) Re-flow the board yourself

c) Replace the board with an Intel based board from R51.

I want to save my t40 
The last time it ran the LCD made checkered 
stripes over a readable desktop. 
I ran it instead on an external monitor which 
seemed to be great. However when I ran it today 
neither the LCD or external would show bios or 
OS screens. Just this mess.

At first I could tell the screen had gotten to the 
windows user prompt since mouse and keyboard commands 
were working. However I'm not so sure anymore
