Mike, I'd download the Hardware Maintenance Manual for you machine and give it a read. I replaced the fan on a T43p - a bit involved (and the thermal pad on the old fan was stuck tight to the GPU) but not too bad with good instructions. Just keep all the screws arranged and don't force things. Make sure you clean off the CPU/GPU if it has old thermal grease. The new fan from Lenovo for the T43p came with new grease already applied in a dot pattern for the CPU and a new thermal pad for the GPU.
The old T43p was WAY quieter with the new fan installed - I expect the bearings in the old one were shot.
