One workaround would be to add the <<setUserName>> macro directly in
your [[MainMenu]] content (or any other tiddler that is always
rendered on startup), so that all new visitors will automatically be
asked to enter a non-default username as soon as they open the
document for the first time.  This ensures that the correct username
will be set *before* any editing can possibly begin (and thus, before
any .lck file would be created), thereby avoiding the problem that you
have described.
