It seems to me the reason you have an "outline" option is because the reference display of links is opaque, at compared to the traditional TOC approach -- you can only drill down to the very next level.
Here's something I'm trying. Certain tiddlers get a tag of "header". If a tiddler appears in a reference listing, and you're using "transclude" mode, it also shows the list of tiddlers that are linked back to it. This allows you to drill down one more level on certain key topics, which is probably enough for most real-life situations.
