You are saving from the local file..
Then you should write:
Name: "name"
Password: "password"
Url of the UploadService script (default: store.php) "http://
name.tiddlyspot.com/store.cgi"
Relative Directory where to store the file (default: .
(downloadService directory))""
Filename of the uploaded file (default: in index.html)  "index.html"
Relative Directory where to backup the file. If empty no backup.
(default: (empty))      "backup"

When you are on your tiddlyspot copy online you simply write
"store.cgi" and not the full path - and you don't specify filename
"index.html"
