In normal cases, TW tries to "protect" content. So if you create a new 
tiddler and the tiddler already exists, a new number will be appended. ... 
BUT this should only happen for "user created" content. ...

So I think you did some experiments, may be with the ActionCreateTiddlerWidget 
... If some variables are not set right, and you try to manipulate the 
$:/StoryList tiddler it might be possible to create several of them. ... 
But that's just a guess.
