I've met a few TiddlyWiki users with files above 5MB, and it definitely works properly with some care. (As you'll have discovered, one issue is that the default sidebar list performance is poor).
So, I'm inclined to suspect one of the plugins that you're using. The intermittent nature of the problem makes debugging difficult, though. It sounds like the problem is during the save operation and I notice that you've got the UploadPlugin loaded which overrides the default save behaviour; it may be worth removing it if you're not using it.
