Hello Gabriel. Did hyou checked the community links? One of the first links shows you how to set up a left menu. If you like some kind of index I think the best option is to tag as index the Tiddlers you want and then use a list filter. Something like
<$list filter="[tag[index]]"> </$list > And include that tiddler as the first one in the default Tiddlers. If you want a desktop wiki, why are you using the node JS version? Don't be afraid of TW.
