You need the null modem adapter which reverses the TX and RX lines on
one end of the cable. Also not all USB to Serial adapters work.. I
bought an old laptop with a real serial port at a junk computer shop
for not much more than USB adapter to solve the RS-232 problem

Try using a plain terminal app and see if you get any weird characters
echo'd back instead of expected responses. Etc... There are a lot of
possible causes when a serial connection isn't working right.
