Many times during development we have had this situation and have learnt that 
we always need a backup of important data when adding new functionality - even 
if it's functionality that should work and, even, has worked before.

We haven't characterized the problem, but there have sometimes appeared to be 
conflicts with schedulers at the same time as updating. Perhaps somebody else 
has more insight into this.

Anyway - if the update takes more than a couple of minutes, we abandon it and 
start again on a new database with the same data. Sometimes we can't drop the 
failed database immediately even after a server restart. We always can drop it 
by the next day so, again, we haven't investigated - we just try later.
