You are correct in terms of current understanding.  In some of the old literature though it was of unknown cause and was felt that the inadequate intake was due to neglect.

Actually, I believe marasmus refers to the biological form of malnutrition and progressive emaciation and is caused by an inadequate intake of both protein and calories.
