You are correct in terms of current understanding. In some of the old literature though it was of unknown cause and was felt that the inadequate intake was due to neglect.

My bad! :) I just remember from years working in child protection that the term was used for failure to thrive while the maternal deprivation factor was a totally different syndrome.
