We're talking about serious behaviorists here!
I'm not familiar with the work of the Cone's (they're applied
behavior analysts).
Fred Keller was a grad school buddy of Fred (BF) Skinner, and
(among other things) developed a teaching system which he labeled the
Personalized System of Instruction, based on breaking the course
material into units, having each student work on a unit until it was
mastered at an 'A' level, then move on to the next.
Material was usually presented textually rather than in a
lecture, and the grade was based on the number of units mastered in
sequence, rather than some qualitative measure of performance.

These days PSI systems are often implemented via CAI.
