The alienist refers to those that were treating the "criminally insane".  These 
were typically
considered Forensic psychiatrists of the time.  The term has its origins in the 
UK.  I might also point
out that there is very good historical novel titled "The Alienist" by Caleb 
Carr.  This book describes
the work of an individual during this time period.  A good read.

As to aliensts , my understanding is that it follows from the early view that 
psychiatric patients were in a state of "alienation" and in need of the 
services of an alienist. 

Aside: There is a good historical novel set in New York City titled The 
Alienist by Caleb Carr.
