The alienist refers to those that were treating the "criminally insane". These were typically considered Forensic psychiatrists of the time. The term has its origins in the UK. I might also point out that there is very good historical novel titled "The Alienist" by Caleb Carr. This book describes the work of an individual during this time period. A good read.
