Do your faculty evaluate your department chairs?  If so, do you have a standard 
form that you use?  If you're willing to share it, I would appreciate it if you 
sent it to me at [email protected]<mailto:[email protected]>.  We're in 
the midst of revising our process.

After I have collected them, I will make them all available to the listserv.
