TMDA has two separate parts - the incoming message part and the
outgoing message part.  Neither of them run automatically.

For the incoming part, you run tmda-filter from a .qmail file.

For the outgoing part, you run tmda-sendmail from your email program
(MUA) on the local UNIX box.  If your users don't log in locally, then
you must use tmda-ofmipd in order to have tmda-sendmail run on the
message.

Since you're not doing either of the above things on the outgoing mail
part, TMDA has no way to get involved.  Your users are sending their
mail directly to qmail's SMTP server and out it goes - no TMDA.

Since you have numerous non-local users, I'd look at setting up
tmda-ofmipd.  Then you'll get TMDA involved and can use the outgoing
mail features.
