Well, as far as I can tell you are correct, both files are very similar and are used the same way and with a minor config file change you could swap the use, send auto-confirmations to whitelist and vice versa.
I use both files and set them up the same way it describes in the documentation. I prefilled my whitelist with the 100 or so addresses from my address book before I turned TMDA on so that known ppl would not be hassled by an challange. Occasionally I also move new known good addresses from confirmed to whitelist and clean out addresses that I do not remember from my confirmed file. I do this becuase occasionally when I get a SPAM auto-confirm that gets added to the list I don't want to have to search through a big long list looking for it (it's usually at the bottom, I know). But, hey it's your TMDA and you can use it however you like. If you want one file then only setup one. Perhaps another reason for separating them would be for outgoing tagged messages. Lets say that outgoing message to whitelist users you wanted to do something special to like extend the amount of time for dated addresses so that they would have a longer window of oppurtunity to reply without a challange. You could do this with two separate files. I bet the TMDA creators and other users will also have some valid reasons as well.
