I've been using good ol' Sendmail with TMDA for about a year. It's beautiful. A little tricky to get going the first time around, but once I had the first one nailed down, adding more folks has not been too bad. (And TMDA-CGI makes that even easier, now.)
Hey, I'm trying to figure out how to ask sendmail what version it is. The man page doesn't mention anything, like a "--V" flag. Anyone? Anyway, I'm fairly certain that my sendmail is version 8.something. Nope, my Sendmail setup doesn't do anything with outgoing - only incoming. I'm going to look into tmda-ofmipd for that... if it's not too hard for me. My hunch is that your sendmail setup does not need to be a show-stopper.
