God - sounds like you've lost the battle. Are you saying that even after DNSBL's like proxies.blackholes.easynet.nl, dynablock.easynet.nl, relays.ordb.org & sbl.spamhaus.org *and* SpamAssassin you *still* have 99.9% spam being accepted and queued on your mail server?
