Do you have a /filters directory?  I have a file,  
"~/.tmda/filters/incoming" with the following line it it. 

"from-file /var/qmail/path_to_user/rodimus/.tmda/lists/whitelist ok"

This tells tmda to let accounts in my whitelist through.  Without it, 
every message  gets a confirm notice.
