IMO, challenging an email should be a last resort.  Most spam has a
bogus envelope sender, so challenges to spam may cause innocent
bystanders to receive challenge replies to messages they never sent.

I use various filters to prevent known good (mailing lists, mostly)
and known bad mail (spam with Asian charsets, etc) from entering TMDA,
and other filters in TMDA (check that the envelope sender can recieve
mail, check if the Bogofilter is certain the message is spam) to
prevent challenges from being issued (either by dropping the email, or
holding it, depending on the filter).  I only challenge about 5% of
the mail that enters TMDA.

I agree for the most part though my solution is a bit different, however the original poster was talking about messages in his pending queue, which means they've already been challenged.
