Here's an update, after a slight delay I did receive the message that I
confirmed, but I still get a confirmation e-mail everytime I send a new
message.  Here's the information you requested:

/home/lori# cat .qmail
|preline /usr/local/bin/tmda-filter
./Maildir/

(.qmail-default is a symbolic link to .qmail)

/home/lori/.tmda# ls
crypt_key       filters         lists           logs            pending
responses

/home/lori/.tmda# ls filters

/home/lori/.tmda# ls lists
confirmed                       (ever since my first confirmatin, this file
contains
                                 my e-mail address, i.e. [EMAIL PROTECTED])

/home/lori/.tmda# ls logs
incoming

/home/lori/.tmda# ls pending
1100348960.23367.msg:3,C        1100349206.23456.msg
1100349547.23516.msg

/home/lori/.tmda# ls responses
[EMAIL PROTECTED]  [EMAIL PROTECTED]
[EMAIL PROTECTED]  [EMAIL PROTECTED]

Any ideas?

Do you have a .qmail-default file for the user?
How are you delivering the mail? through tmda-filter or through .qmail?
do you have CONFIRM_APPEND setting defined in your .config file?
please post contents of your .qmail .qmail-default and .config file
