You do not have any filters configured. I think that's what the problem

You should have a file named incoming there.
Here's the files in my filters folder

incoming  outgoing
[EMAIL PROTECTED] filters]#

and here's a portion of the incoming file

[EMAIL PROTECTED] filters]# cat incoming
from-file /var/qmail/mailnames/utkalika.net/rtwick/.tmda/lists/blacklist
drop
to-file /var/qmail/mailnames/utkalika.net/rtwick/.tmda/lists/revoked
confirm
from-file /var/qmail/mailnames/utkalika.net/rtwick/.tmda/lists/whitelist
ok
from-file /var/qmail/mailnames/utkalika.net/rtwick/.tmda/lists/confirmed
ok
-------------------------------------

So, your incoming filter file should also have atleast one line
coresponding to the confirmed file like 

from-file /home/lori/.tmda/lists/confirmed  ok

and then any body who replies to the challenge once, will not get
another confirmation.

This is a place to look for what action tmda is taking on an incoming
email and can give soem hints on what is wrong.
