I have noticed this since I do set QMAILHOST, QMAILUSER and QMAILINJECT=f in my .bashrc.
Are you setting these to incorrect values? If so, why? If not, what's the problem?
What's your DELIVERY setup to be - are you using the sendmail emulation of qmail, or delivering mail outbound from TMDA via SMTP? If the former, perhaps it's actually a qmail "problem"?
