That's the problem; it looks like the 2nd : in the output should be a .:
not:
[EMAIL PROTECTED]:/home/mydomainorg/users/d:kavadas:536:538
but:
[EMAIL PROTECTED]:/home/mydomainorg/users/d.kavadas:536:538

To debug this more, I need some more information:
* When you logged into tmda-cgi, did you login as "[EMAIL PROTECTED]", or
plain "user".
* Does your web-server set the environment variable HTTP_HOST
* Now consider the domain-name that was either part of the username you
used to login, or in the HTTP_HOST environment variable. Is there an
entry that begins with this domain, followed by a colon, in this file:
/var/qmail/control/virtualdomains
* If not, is there a file named passwd.cdb in the home directory for the
domain mentioned above that contains an entry for the username you
logged in as. I suppose at this point, there must be. I think this is
equivalent to asking "did you define this username"! If so, I suspect
that vmailmgr required a "mailbox" parameter be assigned to that user,
and I suspect this parameter has a typo in it, such that what should be
a . is in fact a :
