I have not yet had a need to support IPv6. qmail-smtpd does not need to be patched, AFAIK, since tcpserver handles the TCP part. But yes, I believe ucspi-tcp does need to be patched. A quick search of qmail.org does not list any IPv6 patches that I can see. So I don't know which one is the most commonly used.


qmail-remote will likely need to be patches, since it needs to create outbound tcp connections.
