I just solved this very problem with help from the list, and have been told I am now the official answerer on this problem ;)
Did you both a) add to the ISAPI dialog in internet service manager and b) add isapi_redirect.dll in the registry under Filter DLL's? That is what the docs say, but it is wrong.
