MS SQL Server's adaptor did not impliment the CREATE DB template.

The MS SQL Server templates did not implement the createdb.vm template. This
causes the test suite to fail. This patch impliments createdb.vm with
drop/create SQL statements.
NOTE: For best performance in MS SQL, it is recommended that the DB be created
thru SQL tools to locate the files on the correct disks, etc. Using Torque to
create a DB should be considered a convience only.
