Found what was wrong: apparently, windows shell blocks some of the extensions,
randomly or not, but this can be overridden in registry as:
[HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\Shell
Extensions\Approved]
"{EEE9936B-73ED-4D45-80C9-AF918354F885}"="TortoiseHg"
