I'm no Windows expert, but a quick google search found the following links

This page describes extension mechanism in general. About the approved shell
extensions, it states that you need sufficient privileges to be able to write
there. Of course THG should fail gracefully if it does not have privileges for
this particular operation.

Another option is for the system administrator to manually approve the 
application
by adding the relevant registry key.
