Same setup - SBS on QNAP - and same problem from time to time. I have SB3s and Boom controlled by iPeng. The art sometimes does not appear on iPeng even if I clear the cache. A full scan always fixes it but so does just leaving it until the next new music rescan. Don't think it is the qnap per se but more to do with player (iPeng) caching.
