Alan,

I'm one of the X12 Claim Status work group co-chairs, your assumptions are
correct, only the 276/277 paired 004010X093 (implementation guide
designation) is specifically named under the HIPAA Final Rules. My work
group has already developed a 277 implementation guide in Version 004020
(004020X104) as a triggering mechanism for a payer to request additional
information about a healthcare claim from a provider. This guide is part of
the overall model for Claim Attachments which includes the ASC X12 275
transaction for enveloping the HL7 booklets which were jointly developed
between HL7 and ASC X12. The three components, are at some point in time,
to be named in a Notice of Proposed Rule Making (NPRM) published in the
Federal Register. Both groups were hoping that would have already happened
by now. Finally the work group is currently developing an update for the
Front End Acknowledgement implementation in version 004040. There is a
draft status of the version 003070 available on the WPC site which was
pre-Y2K, but there have been instances in the industry of successful
implementation of that draft IG.

Bottom line, the only HIPAA implementation guide right now is the paired
276/277 as named in the Final Rule. The other business functions for which
there are Claim Status implementation guides are yet to be added to the
HIPAA suite of named transactions.

Great question. We had a discussion on that just yesterday. Page 11
section 1.3 of the 276/277 guide says separate guides are provided for the
unsolicited 277 and the 277 request for additional information. We
determined they were X12 additional guides that we haven't been able to
obtain yet. I haven't found anything within HIPAA beyond the paired
276/277. Something more official would be nice but we have not included the
stand alone 277's in our mandatory list.
