The ISA/IEA are transmission "envelopes".  Within the envelope there can be one or 
more transactions.  The transactions can be off different types.  For the 837, a 
transaction can contain up to 5,000 claims.  So this means you could receive three 837 
transactions, a 276 transaction, a 270 transaction, and a 997 transaction within the 
same envelope from say, a clearinghouse.

The 997 is a non-required companion transaction.  It is common business practice to 
use it, however.  There is also a TA1 transaction to reject really bad or miss routed 
transmissions and a discussion is ongoing regarding the use of the 824 to handle HIPAA 
compliance rejects.

Well, I thought I was beginning to understand transactions when I ran across the 
ISA/ISE set.  It appears, from what I read, that this wraps all transaction sets so 
that, for instance, a set of 837 transactions will begin with an ISA record and end 
with an ISE record.  

Is this true?

Also,  although the 997 appears not to be mandatory, it sure is useful.  Are all/most 
receivers going to implement this?
