What guarantees that the transaction sets will be hierarchically compliant?
For example: Scenario A Billing Provider/Subscriber/Patient/Billing Provider/Subscriber/Patient ... versus Scenario B Billing Provider/Billing Provider/Subscriber/Subscriber/Patient/Patient ...
