276 Bill Type

The 276 IG on page 80 talks about the Institutional Bill Type Indentification and says that it refers to CLM05 in the 837. CLM05 has sub-elements.  Which sub element is the IG referring to? The example in the 276 lists REF*BLT*111~ 111 is three bytes, the subelements in CLM05 can only be 1 or 2 bytes long.
