me are the companion documents from several
FIs.  And they read virtually identical to one
another.

Brian, what is the legislative justification for a CMS FI ignoring the PWK
segment?  I just checked the HIPAA IG, it says the segment is required if
there is paper documentation supporting the claim.  I also checked the
addenda to the addenda, and there is nothing reversing that.  So, how can
CMS justify ignoring the segment while still requiring a paper attachment to
support the claim?
