Unfortunately this Japanese translation looks very bad, containing grammatical errors and unusual expressions that require time for Japanese speakers to understand. If you are going to use as it is, I would guess that this would hurt participation to the survey, as well as the quality of the answers. If there is any raw material of this translation available, I would like to help improving it.
