I tried to read the source code to figure out some of this stuff

I think this means "Request token sent" (during the login handshaking)
in the context

The %s is the command name, and the %02X is the command code. I think
this means an error reply has been received for the command that has
been sent.

This appears in a function called
_qq_search_before_add_with_gc_and_uid, so I think this means "Do you
want to add this user to your buddy list?"

> "Would you add?"        Incomplete, or just very short?

The context is "the person is not in my list", so I think this means
"Would you like to add him/her to your buddy list?"

The 'net used to be run by smart people; now many sites are run by
