There is one in the Cocoa Cinnamon on Hillsborough Rd in Durham, but for that one i took the insides out and wired everything directly to the receiver/dial. The newer version (pictured and which i'm trying to make a board for) uses a SLIC board so an unmodified phone can plug into it. I'm sure i will get it wrong a few times before i get it right. :)
