If Reply-To is set in the sender's client, then the message will reply to the list (from the list default) and the sender (from his/her client). Some clients set the users email address as the Reply-To object automatically, but some don't. Whether or not they receive two emails depends on their mail server, but most mail servers these days figure it out and only send one message. Generally, Reply-All just collects or retains people in the CC boxes and adds them to the return distribution.
