It really depends on a lot of things. It's possible there's something physically wrong with your cables that's not letting you get all the bandwidth you should be able to. It could be random cosmic rays. Basically, it could be anything.
That said, the setup I use is exactly the same one that Jason mentioned: the cable modem connected to a WRT54GS connected to the vonage router. In addition, I don't have anything else connected to the vonage router, the rest of my network is connected to the WRT54GS. I am running the latest free Sveasoft firmware and have setup QoS and have yet to notice any problems with packets being dropped.
