Linux build looks fine now and Calculator and BigBank work (though BigBank
failed the first time I ran it).

Windows build is lacking the ws_reference/ws_service library builds. I'll
add these tomorrow. Exceptions need some rework to catch the
TuscanyRuntimeExceptions in the cpp extension code and rethrow as
osoa::sca::ServiceRuntimeExceptions.

 Hopefully tomorrow it will be fairly stable. In the next few hours it
will be highly unstable as I check in today's refactor.
