I'm about  99.5% positive the "running across stage" deal predates the bypass.  Didn't "the new intro" (that only appeared once) come before the surgery?  There was a shot in there of Dave in silhouette running past the front door of The Ed, as the camera panned across.  He was definitely doing it at least that far back, and probably several years prior.

My theorey has been that he started doing the running thing after he recovered from his heart operation (Donz, do you have any opinion on whether that is factually correct?) both showing that he was healthy again and getting him pumped for the show.
