Wait...hold the phone. I'm thinking of the wrong intro. Not only was it not the "one-day-only" intro, it was the previous intro, that was playing at least as recently as 1997 (according to an old video I have in the collection). The very last shot in that intro, was Dave running past the front door of The Ed as the camera swooped in towards the door, as if to continue inside, at which point the shot changed from the intro to the live shot of the current show's stage.
I can't confirm it yet, but I believe that's the same intro that was used from the beginning of the move to CBS, which would date the "running across stage" thing to at least 1993. But, even if I can only confirm it back to 1997, it easily predates Dave's surgery in 2000.

I'm about 99.5% positive the "running across stage" deal predates the bypass. Didn't "the new intro" (that only appeared once) come before the surgery? There was a shot in there of Dave in silhouette running past the front door of The Ed, as the camera panned across.
