That kind of thing can still happen. A couple of years ago there was a famous instance in the UK when a live match on ITV went into extra time (overtime). All of a sudden, the automatic ad-playout system cut in after a couple of minutes of action, and before anyone could manually cancel that, viewers had missed a vital goal. When the footage cut back, players were celebrating.
ITV was very apologetic after the event and promised to put new procedures in play to stop automatic commercials being run during live sports events.
