I think the issue is more about potential logistics problems than medical threats to the contestants. Countries are revising their regulations about who can enter and leave almost daily. If a challenge is in Italy, all the infrastructure is set up for it, and at the last second the government decides to close entry until further notice, the production is screwed. Likewise if the contestants could get into Italy and complete the challenge, and then fly to another country where they are put into 14 day quarantine, the production is screwed.
