Need some help building a page

Hi every, I need to build a page like this: 
http://www.dropmocks.com/mBjQmW and have a online example here 
http://soporte.maxcenter.com.ve/home/comment but because forms can only 
have classes like: horizontal and vertical I didn't know how to put the 
first column span with elements align to left before inputs and in the 
second before the input at top. Any help or advice?
