Wow, you're good.  I did a CNAME to change the file name to HS_FILE_ACCESS.OLD and 
then ran option 3 from the HS.ADMIN menu - this time it created the file and added the 
new entry, without issue.  So thanks. :o)

sounds like the file already exists at the os level in that account - try
deleting it 1st.
