For sites: uvbackup to local disk image to ensure database image consistency. Then OS backup to include the disk image.
For home: I run virtual PCs for everything: as well as being extremley useful for testing various configurations, they are hardware independent so VPC images can move between machines with no problems. I run windows scripts to zip up the VPC disk images at end of day and copy them to a safe directory on my laptop (available for immediate extraction if I mess up) and also to a backup server. These are copied to data keys, committed weekly to DVD and a spare copy sent to my folks, who have a copy of virtual PC loaded, in case of disaster. Incidentally, two weeks ago I had to send my laptop (which has all my development work) off for repair. With VPC I was up and running on a spare laptop in under 2 hours :) VPC rocks.
