One reason for why the upper case only thing started that has been mentioned in this thread is that some early terminals didn't have lower case.
This is indeed true but it used to be much worse than that ! The early days computers themselves didn't handle lowercase as they had 6bit characters allowing for a 64 character alphabet only. I've only used two such machines the IBM 7044 and CDC 6600 but before the IBM360 arrived in the mid 60's it was the general rule for mainframes to have 6bit chars. ( There were exceptions.
